Ambulatorio della Sclerosi Multipla di Frosinone

La Sclerosi Multipla ? una malattia infiammatoria cronica demielinizzante che colpisce il sistema nervoso centrale (cervello e midollo spinale). La grande variabilit? dei sintomi che la caratterizzano ? conseguenza di un processo di degenerazione della mielina, da qui il termine demielinizzante. La mielina costituisce il materiale che riveste i nostri nervi, permettendo la trasmissione rapida ed integra degli impulsi nervosi. Nel corso della malattia la distruzione delle guaine mieliniche causa il blocco o rallentamento degli impulsi che vanno dal sistema nervoso centrale verso le diverse parti del corpo e viceversa. Le aree in cui la mielina ? stata danneggiata vengono anche dette placche. Da qui l'altro nome della malattia, "sclerosi a placche". Se in uno stato di normalit? le informazioni nei nervi sono trasmesse a 100m/s, in un individuo affetto dalla Sclerosi Multipla la velocit? scende gradualmente a 5m/s Non si riesce ad identificare in un solo agente la causa determinante la Sclerosi Multipla, ma alcuni dati epidemiologici devono essere tenuti in considerazione:

La Sclerosi Multipla colpisce circa tre milioni di persone nel mondo, mezzo milione in Europa e pi? di 50.000 in Italia. Tra i disturbi neurologici ? il pi? diffuso tra i giovani adulti e la principale causa neurologica di disabilit?. La malattia si presenta primariamente in et? compresa tra i 14 e i 40 anni, con un picco verso i 30 anni. ? invece poco frequente sopra i 50 anni. Colpisce le donne con una frequenza di 2,5:1 rispetto agli uomini. L'incidenza ? inferiore a 1/100.000 nelle aree equatoriali, varia da 6 a 14/100.000 nel sud degli Stati Uniti e nell'Europa meridionale e da 30 a 80/100.000 in Canada, nell'Europa settentrionale e nel nord degli Stati Uniti. Ci? ha messo in evidenza l'esistenza di qualche fattore ambientale, ancora sconosciuto, che contribuirebbe all'insorgenza della Sclerosi Multipla indipendentemente dalla razza. Numerosi studi hanno evidenziato che persone che migrano da una zona ad alto rischio ad una a basso rischio portano con s?, almeno in parte, il rischio correlato al loro luogo di origine. Ci? avviene nel caso in cui il paziente in questione emigri dopo i primi quindici anni di vita. Prima di questo lasso di tempo, infatti, si tende ad assumere il rischio presente nella zona verso cui si emigra.

Attualmente si ritiene che possano esistere fattori ereditari, in seguito alla scoperta che alcuni antigeni di istocompatibilit? (HLA) sono pi? frequenti nei pazienti affetti da Sclerosi Multipla rispetto ai soggetti di controllo; questa ipotesi tuttavia non spiegherebbe perch? emigrando da una zona a bassa incidenza verso una ad alta incidenza, si tende ad assumere lo stesso rischio dell'area di arrivo, prescindendo dai caratteri genetici della popolazione da cui si proviene. Ad ogni modo la componente ereditaria della malattia ? evidente visto che il rischio di ammalarsi risulta maggiore nei parenti stretti di persone affette da Sclerosi Multipla. Altre ipotesi, meno frequentate dalla scienza medica, ritengono il vaccino contro l'epatite B (una sostanza a base di alluminio) una possibile causa dell'insorgenza della malattia. Il fatto che la Sclerosi Multipla insorga in zone dai consumi uguali (occidentali) e sia invece sconosciuta nei Paesi vicini all'equatore, confermerebbe il collegamento con i consumi. L'alluminio, iniziato a produrre industrialmente nel 1902, potrebbe avere una relazione con le proporzioni epidemiologiche che la Sclerosi Multipla ha avuto nel ventesimo secolo.

Come la scorsa edizione, anche quest'anno il link "banca" oltre a fornire informazioni sulla storia, il gruppo, la privacy e un elenco di comunicati stampa, mette a disposizione del pubblico la possibilit? di visitare alcuni palazzi storici di propriet? della BPN quali, il Bellini e l'Altieri, vedere la collezione dei coralli Siciliani e dei quadri. Il link "Banca on line" propone i Servizi On-line della Banca quali, home banking, wap, sms, PMI on line, Pronto BPN. Soltanto il servizio di home banking privati ? dotato di una demo.

Anche quest'anno ? stato rilevato lo stesso difetto nella descrizione dei prodotti bancari, e cio? l'assenza di trasparenza: mancanza dell'indicazione dei costi (disponibili solo sul servizio home banking e wap), dei tassi, della descrizione analitica del prodotto e della possibilit? di avere informazioni aggiuntive (tramite la possibilit? di scaricare manuali, contattare responsabili, inviare mail); infatti al termine di ciascuna scheda, la banca invita a consultare gli appositi fogli analitici presso le filiali.

Ci sembra inoltre molto strano il fatto che i primi tre link della sezione "Servizi", siano ancora in fase di revisione e di realizzazione (come lo scorso anno).

La banca dedica anche una sezione alle news on-line in particolare sono presenti i comunicati stampa ( ma solo del 2001/2002), ed una parte dedicata alle news dove sono descritte le novit? della banca stessa.

I prodotti offerti ai clienti sono suddivisi in base ai soggetti cui sono destinati cio?: privati, P/M imprese, Aziende. Claudio Ferrarini Flauto Proporre programmi originali che sconfinano dal solito repertorio non è impresa facile, soprattutto se lo si fa pensando ad un debutto per una grande casa discografica: è invece quello che avviene per la presente registrazione che vede impegnati l'ormai affiatato duo composto dall'arpista Floraleda Sacchi e dal flautista Claudio Ferrarini, entrambi concertisti affermati internazionalmente e dalla solida esperienza. Il viaggio che il duo Sacchi-Ferrarini propone all'ascoltatore è tutto incentrato sul Novecento musicale e di questo vengono presentate le diverse tendenze, scuole di pensiero, legami reciproci ed influenze. Il mezzo è rappresentato dal duo flauto/arpa, una delle combinazioni strumentali più affascinanti e capaci di generare atmosfere sonore da sogno: già se ne era accorto Mozart che per tale combinazione strumentale aveva composto lo splendido doppio concerto per flauto, arpa e orchestra KV 299, indimenticabile soprattutto per il suo secondo movimento, l'Andantino. Il programma si apre con la deliziosa trascrizione della Gymnopédie n. 1 di Erik Satie ). Satie fu autentico precursore di movimenti musicali a lui più tardi quali ad esempio il Minimalismo e certe atmosfere New-age sempre più care oggi nell'era del "fast and easy-listening". La presente trascrizione, dall'originale per piano, risulta perfettamente riuscita tanto da sembrare quasi pensata appositamente per tale formazione; del resto, la concezione predominante della melodia in Satie trova nel suono del flauto, cui è affidata la linea della mano destra del piano, la sua più completa e totale esplicazione.

Dalla Francia del primo novecento si passa alla prima registrazione mondiale del brano di Philip Glass (1937) Open the Kingdom per la formazione flauto/arpa. Brano originale del 1985, su testo di David Byrne, rappresenta un primo importante punto di arrivo della concezione estetico-musicale di Glass fondata sul Minimalismo, sulla ripetizione cioè incessante di cellule melodico-ritmico-tematiche. Si prosegue poi con il celebre Histoire du Tango di Astor Piazzola ). Composto originariamente per flauto e chitarra e dallo stesso Piazzola poi trascritto per flauto e arpa, questo brano è forse il pezzo forte dell'intero Cd, tanto è piacevole all'ascolto questa storia sintetica dell'evoluzione del tango. Molto bella l'interpretazione da parte del duo Sacchi-Ferrarini che dimostra un'assoluta padronanza degli stilemi propri di questo repertorio dove ritmo, colore del suono, articolazione ed agogica devono sapersi fondere reciprocamente.

Si prosegue con un altro autore francese, questa volta meno conosciuto ma di sicuro interesse quale Jean Cras ), ufficiale di marina che divise la sua vita fra la carriera militare e la musica. Di Cras il Cd propone il brano Suite en Duo, che dà il titolo all'intero album Philips. In quattro movimenti, la Suite en Duo rappresenta al meglio l'arte di Cras, basata sul costante richiamo a ritmi e suoni sentiti durante i suoi numerosi viaggi per mare (in questo caso i ritmi e i suoni della Guinea) con elementi propri della tradizione bretone. Anche in questo caso Sacchi e Ferrarini si mostrano profondi conoscitori di tale repertorio che necessita di grande leggerezza e padronanza tecnica per rendere nella sua totalità le sfumature timbriche di cui si nutre questa immaginifica musica.
